Episodes are made in blocks, and often will get aired out of sequence. (As, indeed, happens at the start of second season, where The Cyborg is plainly meant to introduce the Flying Sub, but And Five of Us are Left, screendated before it, also precedes it in airdate.)
As the dated episodes on Voyage are all over the shop anyway (it's one of the reasons I feel justified in treating Season 1 as a separate series - I did once try to get them into internal chronological order - I must have been mad), and the fact that Mr Allen cheerfully resused already aired scenes to pad out episodes when they were behind hand, I'm not going to get too worried about the three star/four star switch...
